Special Needs plans from UnitedHealthcare: I-SNP and IE-SNP

A Special Needs plan (SNP) is a Medicare Advantage plan designed to provide targeted care for qualified individuals.

What is an I-SNP?

 

An Institutional Special Needs Plan (I-SNP) is for people who need the level of services provided in a long-term care facility. You must be a Medicare beneficiary entitled to Medicare Part A and enrolled in Part B, and a long-term resident of a participating nursing home, to enroll in an I-SNP.

What is an IE-SNP?

 

An Institutional Equivalent Special Needs Plan (IE-SNP) is for someone who needs the same nursing home level of care, but can live either at home or in an assisted living facility. This is still considered an I-SNP. You must be a Medicare beneficiary entitled to Medicare Part A and enrolled in Part B and live in a plan service area to enroll in an IE-SNP.

The plan includes an annual routine hearing exam plus hearing aids every two years.

Routine podiatry

Benefits include foot care visits for nail trims and other covered preventive care.

Dental coverage

Network preventive dental including exams, x-rays, routine cleanings and fluoride.

Transportation assistance

Members receive transportation to or from doctor visits and the pharmacy.

    OTC benefits have expiration timeframes. Review your Evidence of Coverage (EOC) for more information. Routine transportation not for use in emergencies. A trip is one-way and roundtrip is two trips. The plan only covers hearing aids from a UnitedHealthcare Hearing network provider. Other hearing exam providers are available in the UnitedHealthcare network. If your plan offers out-of-network dental coverage and you see an out-of-network dentist, you might be billed more. Network size varies by local market. Annual routine eye exam and an allowance for contacts or one pair of frames, with standard (single, bi-focal, tri-focal or standard progressive) lenses covered in full every year. 

     

    Plans are insured through UnitedHealthcare Insurance Company or one of its affiliated companies, a Medicare Advantage organization with a Medicare contract. Enrollment in the plan depends on the plan’s contract renewal with Medicare. The plan is for eligible Medicare beneficiaries living in a participating nursing home for over 90 days.

The healthy food benefit is a special supplemental benefit only available to chronically ill enrollees with a qualifying condition, such as diabetes, cardiovascular disorders, chronic heart failure, high blood pressure and/or high cholesterol, and who also meet all applicable plan coverage criteria. There may be other qualified chronic conditions not listed. Routine transportation not for use in emergencies. A trip is one-way and roundtrip is two trips.
